All visits have to be booked on this page. Once I view your appointment request, I will email you a confirmation with the details about your appointment, intake and consent forms you will need to fill out prior to your appointment. Please don’t hesitate to contact me if you have any concerns or questions about your upcoming visit.

I also have limited availability for home visits based on special circumstances and if you live within 25 minutes driving distance from New Hartford. A travel fee will be applied to your visit. (up to 10 miles $30, 10-25 miles $60) Please text us at 315- 497-7080 to inquire.
