All visits have to be booked on this page. Once we view your appointment request, we will email you a confirmation with the details about your appointment, intake and consent forms you will need to fill out prior to your appointment. Please don’t hesitate to contact us if you have any concerns or questions about your upcoming visit.

We now offer a limited number of discounted self pay home visits  if you live within 25 minutes driving distance from New Hartford or Mohawk. A travel fee will be applied to your visit cost. (up to 10 miles $30, 10-25 miles $60) These visits will be scheduled with our Intern Brittney Watkins, CLC. View our Self Pay rates here.
